Discharge planning begins prior to, or at the time of, admission to the hospital.  Specially trained staff known as 'Case Managers' collaborate with the patient, family members, physicians and the health care team and community resources (home health agencies, rehabilitation facilities, etc.) to ensure that everyone knows the 'game plan'.  The goal is to eliminate delays that may occur as a patient is transitioned from the hospital to their home or to another health care provider.
